What is it about?
The cohousing of Fondazione Maddalena Grassi is a form of shared housing in which a group of people live in a community that combines private spaces and common areas, with particular attention to healthcare.
Guests are offered support in carrying out daily life, through periodic interventions by multi-professional operators, personalized rehabilitation programs and possible attendance at the Day Center.
ACTIVITIES
Basic activities
• Objective: Maintain or enhance daily skills.
• Activities: Self-care, management of personal and community environments.
• Purpose: To enhance the times and environment of life.
Body Activities
• Objective: Promote physical well-being.
• Activities: Gentle gymnastics, walks.
• Purpose: Promote self-acceptance and physical well-being.
Expressive Activities
• Objective: To test manual and cognitive skills.
• Activities: Creative workshops, art therapy, theatre.
• Purpose: Develop creativity, improve self-esteem and confidence in one's abilities.
Resocialization and Group Activities
• Objective: Improve social interactions and community climate.
• Activities: Group counseling, social meetings, outings and trips.
• Purpose: Promote positive ways of comparing and processing experiences.
Activity "Below - above"
• Objective: Participate in specific themes.
• Activities: Variable themes based on the time of year or events.
• Purpose: Engage the community in pertinent and relevant activities.
Organization
• Scheduling: Defined annually.
• Participation: Mandatory, considered essential for a positive community climate.
Who is it aimed at?
It is aimed at people who, after a rehabilitation process, have recovered good relational competence and a degree of autonomy such as to allow social, housing and work recovery.
They are offered support in carrying out daily life, through periodic interventions by multi-professional operators, personalized rehabilitation programs and possible attendance at the day centre.
Delivery method
The guest is expected to contribute to the social/hotel expenses.
Activation of the service
Identification of the need for hospitalization
The Social Services or the Hospital, having identified the need for hospitalization of the patient at the Housing Home, forwards a request to the Management for an evaluation. The report includes:
- AIDASS form (GLOBAL FUNCTIONAL ASSESSMENT OF AIDS PATIENTS)
- Infectious disease report
- Psychiatric report (if necessary)
- li>Social Service Report
Informative interview
If the application is accepted*, a knowledge interview is organized (in the facility or at home or in the hospital, if the patient is unable to present himself) which is carried out by the management of the Accommodation House with the presence of any family members and with the contact person of the sending service.
On this occasion, the date of inclusion in the structure is decided.
*The entry is subject to the availability of beds and the satisfaction of the admission requirements.
Observation period
The patient is welcomed into the facility and will share an observation period lasting one month with the operators.
Definitive Educational Project
At the end of this period, by mutual agreement between operators and guest, the definitive Educational Project is formulated and the guest officially enters the Accommodation House.

The structures
The high-intensity care home is located in Baruccana di Seveso, in via Meredo 39. It hosts 10 people on a residential basis and 2 people on a day basis. It has an agreement with the ATS of Monza-Brianza.
Today it is the second most important home in Lombardy for people affected by HIV.
Opened in 1994, offers Home Accommodation and Cohousing services. u>
