What are they

Home palliative care is a type of care that aims to improve the quality of life of the patient and his family unit through assistance capable of meeting health, social, psychological and spiritual needs.

< p>The objective is to respond to the needs of the patient in the advanced and terminal stages of the disease, in collaboration with the other structures of the local Palliative Care network, with the General Practitioner, the Free Choice Pediatrician and with the family.

Who are they addressing?

They are aimed at people of any age who are treatable but not curable.

Delivery method

Home palliative care (CP-DOM) can be activated in agreement with the Regional Health Service (SSR) or privately.

Activation of the service

Prescription
The indication to activate home palliative care may come from:
  • the patient’s general practitioner
  • the doctor of the hospital department at the time of discharge
  • The specialist doctor who is treating the patient
Attention: it is necessary for there to be a person (care giver) at home dedicated to caring for the patient. The caregiver is a family member who experiences the care process together with the patient. He or she plays an informal role of assistance, support and closeness, actively participating in the experience of the disease and dedicating himself or herself to the daily activities of caring for the person.

If you or a family member has been advised to activate home palliative care (CP-DOM) what you need to do is contact one of the facilities accredited for provision directly.

Through a preliminary interview, the facility will evaluate whether the patient’s conditions correspond to those specific for taking charge.

Fondazione Maddalena Grassi, our professionals will schedule a preliminary interview with the caregiver followed by the first evaluation visit. Then, they will define the Intervention Plan (I.P.) and the Individual Assistance Plan (I.P.), activating the taking charge of the patient.< /p>

(The Foundation is responsible for the healthcare material necessary to carry out the services provided for in the plan. The service is totally free for the user).

The headquarters in via Bordighera is also the operational headquarters for the ‘ATS of Milan Metropolitan City.

The Palliative Home Care service (CP-DOM) of the Maddalena Grassi Foundation is authorized and accredited by the Social and Health System of the Lombardy Region.

It currently operates in the territory of the ATS Milan Metropolitan City in the following ASSTs :
